Mathew was talking about headroom problems. As a pressurized tube, the
fuselage is most efficient with a circular cross-section.  If the
diameter is too small, the designers must start reducing headroom from
the passenger cabin or the cargo cabin... the former may be
uncomfortable and the latter is downright costly.
