Actually, if you're talking about the A330-300 and A340-300, there'd be
no need to change the fuselage.  The differences are the center main on
the A340, some localized structural differences in the area of the
engine mounts, and a hell of a lot of wiring and other systems.  That
last part along with the cost of the engines would undoubtedly make such
an exercise prohibitive.
