There's an urban myth (who knows, it may be true) that the one-time chairman of Rolls Royce, a company that make aero engines, always flew the atlantic in 4-engined planes. When asked why, he replied "because there are no 5-engined planes." More seriously, ETOPS regulations include things like upgraded capability for the suppression of fires in the hold that make them safer, in that respect, than 3- and 4-engined planes.
